Northern Health Centers has one full-time social worker, one patient financial advocate, and currently one certified application counselors. All are here to guide you through locating, understanding, and obtaining healthcare benefits. Our staff is knowledgeable about the healthcare policy changes, insurance, and prescription drug programs. Our team can screen for potential eligibility in various programs and assist in submitting an online application for the Marketplace Insurance Exchange and other programs. Sliding Fee Program

Northern Health Centers is a Community Health Center (CHC). One of the benefits of being a CHC is that we are able to offer a Sliding Fee Scale benefit to each and everyone of our patients that walks through our doors. Our sliding fee scale program is a discount program that allows us to adjust your charges based on your income status and household size. The sliding fee program allows us to reduce or “slide” the fees for the services you or your family receive at NHC. You can apply for the program if you need assistance to help you pay for your care. We are audited yearly for this program and need to be able to show that we offered it to all of our patients. Therefore it is important that you complete the application if you are interested in applying for the benefit, as well as provide the requested proof of income. You absolutely have the right to refuse the program if you wish.

Frequently Asked Questions

Is NHC a free clinic?

No. All patients are responsible for a portion  of the cost of their care.


What if I have no income?

You can still apply for the program. Our Patient Financial Advocate will be able to assist you with the appropriate paperwork. We may also be able to help connect you with other community/state benefit programs.


How often do I have to apply?

It is required that you update your application annually. However, you can update your application any time your circumstances change.


What if I have insurance?

While it is your right to decline the program it may be to your benefit to apply anyway. The Sliding Fee Scale program can assist you with charges not covered by your insurance or with your deductible.
