Douglas Gremban, MD - Dr. Gremban has been board certified with American Board of Family Practice since 1990. He has a medical degree from University of Minnesota School of Medicine, Minneapolis, Minnesota (1978). Residency completed with Family Practice Program, Midland Hospital Center, Midland, Michigan (1981). He has practiced medicine in surrounding areas that include: Eagle River, Phelps, Appleton, Wausau, Oconto Falls, Suring, Lakewood, and Mountain.Dr. Gremban has been Medical Director of various clinics and nursing homes over the years. Dr. Gremban will see newborn, pediatric, adult, and geriatric patients.

Philosophy: "I've learned over the years that in order for the pateint/physician relationship to work well, both parties have to take equal responsibility in the treatment and follow up of medical issues. I also believe in taking the time to listen to patients. By getting a good history, a physician should be able to come up with a diagnosis of any medical problem 90% of the time."

Leslee Granke, RN, MSN, WHNP -Leslee received her Bachelor of Science degree in Nursing from Oakland University in Rochester, Michigan in 1978 and her Masters of Science degree in Nursing from the University of Wisconsin Milwaukee in 2000. In 1992 Leslee earned her certification as an Advanced Practice Nurse Practitioner in Women’s Health. Leslee specializes in female adolescent health, all aspects of family planning, bladder and menstrual problems, pelvic pain, diagnosis and treatment of STD’s menopausal concerns, headaches, depression and anxiety diagnosis and treatment. Leslee enjoys counseling her patients in preventative health. Including her patients in their health care decisions is very important to her. In her spare time, Leslee enjoys organic gardening, cooking, walking and all aspects of being a grandmother!




Gretchen Tolsma, DNP, APNP - Dr. Gretchen has been in primary care for over 20 years completing all of her education in the great state of Wisconsin. She attended Marian University for her undergraduate education and obtained her Master’s degree as a Nurse Practitioner from Marquette University. She went on further to earn her Doctorate of Nursing Practice from Concordia University. Dr. Gretchen has worked in cardiology with a focus on congestive heart failure and completed her residency with a focus on pulmonology. She has worked in rural Wisconsin as an independent provider for 5 years and also had an independent practice for 8 years in the town of White Lake, WI. Dr. Gretchen enjoys spending time with her family, grandchildren and horses. In her free time she loves to garden and travel. Her favorite places to be are on the water in the warmer months or driving rough terrain in her Jeep! You can find Dr. Gretchen at both our White Lake and Lakewood Clinics. As a certified DOT Exam provider and with her many years of experience in her field, Dr. Gretchen is a great asset to NHC's medical team.
